A sperm analysis (semen analysis or spermiogram) is the single most important first test for assessing male fertility. In Turkey it is performed the same day in an accredited andrology laboratory using World Health Organization (WHO) reference standards, typically from around £60. A sperm analysis is a laboratory examination of a fresh semen sample that measures the quantity and quality of sperm. It is the cornerstone investigation when a couple is having difficulty conceiving, because male factors contribute to roughly half of all fertility problems. The test reports on semen volume, sperm concentration (count), total sperm number, motility (how well sperm swim), progressive motility, morphology (the percentage of normally shaped sperm), vitality, pH and signs of infection or inflammation.

Results are compared against the WHO 6th edition (2021) reference values — for example, a semen volume of at least 1.4 mL, a concentration of at least 16 million sperm per millilitre, total motility of at least 42% and at least 4% normal forms. These are lower reference limits, not pass/fail marks: a result below a threshold flags an area to investigate, while a result above it does not by itself guarantee fertility.

You provide a semen sample by masturbation into a sterile container, ideally in a private collection room at the clinic so the sample reaches the laboratory quickly and is kept at body temperature. For an accurate result you are asked to abstain from ejaculation for 2 to 7 days beforehand, avoid alcohol and acute illness, and report any recent fever or medication. The whole visit is brief, private and completely painless.

In the laboratory the sample is allowed to liquefy, then examined under a microscope and, in many clinics, with a computer-aided semen analysis (CASA) system for objective measurement. The technician counts sperm, grades their movement, assesses their shape and checks for white blood cells, agglutination and other abnormalities. Depending on findings, the clinic may add specialist tests such as sperm DNA fragmentation, an anti-sperm antibody (MAR) test, a semen culture for infection, or a post-ejaculatory urine test if retrograde ejaculation is suspected.

A sperm analysis is suitable for any man who wants to assess his fertility: couples trying to conceive for 12 months without success (or 6 months if the female partner is over 35), men with known risk factors such as a previous varicocele, undescended testicle, mumps, chemotherapy or testicular injury, men preparing for IVF or ICSI, sperm donors, and those confirming the success of a vasectomy or a vasectomy reversal. It is a diagnostic test with essentially no medical contraindications, so almost anyone can have it. The main caveat is timing: results can be temporarily affected by recent fever, illness, stress, alcohol or too short or too long an abstinence period, which is why a single abnormal result is rarely conclusive on its own.

There is no recovery period. A sperm analysis is a non-invasive diagnostic test, so you can return to work, travel, exercise and all normal activities immediately afterwards.
