Overview

Smart Lens, also called refractive lens exchange or lens replacement, replaces the eye's natural lens with a premium artificial intraocular lens to correct long-sightedness, short-sightedness and especially presbyopia, while removing the future risk of cataracts. In Turkey both eyes are typically treated on separate days during a 5-7 day stay, from around £2,500 for both eyes. It suits people roughly 45 and over who want lasting freedom from reading glasses and varifocals, and who may not be ideal candidates for laser eye surgery. Smart Lens, known medically as refractive lens exchange (RLE) or clear lens extraction, is essentially the same procedure as modern cataract surgery — but performed before any cataract has formed, purely to correct your focus. The surgeon removes the eye's natural crystalline lens and replaces it with an artificial intraocular lens (IOL) chosen to match your prescription and lifestyle. Because the natural lens is removed, you can never develop a cataract in that eye afterwards.

The implant itself does the focusing work your glasses or contact lenses used to do. Depending on the lens type — monofocal, extended depth of focus (EDOF), or multifocal/trifocal — it can give clear vision at distance, intermediate (computer) and near (reading) ranges. 'Smart Lens' is a marketing name for the premium trifocal and EDOF lenses most often used.

How it’s done

The procedure is performed under topical (eye-drop) anaesthetic, usually with mild sedation, and takes around 15-20 minutes per eye. Through a tiny self-sealing incision of around 2-3 mm, the surgeon uses phacoemulsification — fine ultrasound — to gently break up and remove the natural lens, then folds the chosen intraocular lens through the same opening and positions it inside the lens capsule. No stitches are usually needed, and the two eyes are normally treated on separate days so each can be checked before the second is done.

Before surgery you have detailed biometry and scans that measure the length and curvature of your eye, map the cornea and check the retina and macula. These measurements determine the exact lens power. Your surgeon discusses realistic expectations and the trade-offs of each lens type, particularly night-time halos and glare with multifocal lenses.

Is it right for you?

Good candidates are usually 45 and over, frustrated by reading glasses or varifocals, or with high long-sightedness, high short-sightedness or early cataract changes — and people whose cornea is too thin or prescription too strong for laser eye surgery. A healthy retina, macula and optic nerve are essential. It is generally not suitable if you have significant macular degeneration, advanced glaucoma, certain retinal disease or unrealistic expectations of perfect spectacle-free vision in every situation. Younger patients with healthy lenses are often better served by LASIK or implantable contact lenses; the assessment exists to steer you to the safest option, not to sell you a lens.

Recovery & aftercare

Most people notice clearer vision within a day or two, though it is normal for sight to be blurry, watery or sensitive to light for the first 24-48 hours. You will use anti-inflammatory and antibiotic eye drops for several weeks and should avoid rubbing the eyes, swimming, dusty environments and heavy exertion for the period your surgeon specifies. Driving usually resumes within a few days to a week, once your vision is legally adequate and comfortable.

Full visual adaptation — especially the brain learning to use a multifocal or trifocal lens for both near and distance — typically takes several weeks to a few months. Night-time halos and glare are common at first and usually fade as you adapt. A 5-7 day stay in Turkey allows your first post-operative checks before flying home; remaining follow-up is coordinated remotely or with a local optometrist.

Results to expect

Many patients achieve good spectacle-free vision for everyday activities, and most substantially reduce their reliance on glasses. With trifocal and EDOF lenses a high proportion read, use screens and drive without glasses, though some still prefer glasses for very fine print or prolonged night driving. The correction is stable and permanent because the artificial lens does not age or cloud like a natural lens. No surgeon can guarantee complete independence from glasses in every situation, and individual results depend on your eyes, lens choice and healing — your surgeon will give you a realistic, personalised expectation.

Safety & honest risks

Smart Lens is a well-established procedure, but it is intraocular surgery and carries real risks that should be weighed carefully. These include infection (endophthalmitis, rare), inflammation, raised eye pressure, swelling of the macula, and in a small number of cases retinal detachment — a risk that is somewhat higher in very short-sighted eyes. Multifocal and trifocal lenses can cause halos, glare and reduced contrast at night, which a minority find bothersome. Sometimes a residual prescription needs fine-tuning with a laser touch-up, and months or years later the lens capsule can cloud over, treated quickly with a simple YAG laser. Reputable clinics mitigate these risks with thorough screening, sterile technique, appropriate lens selection and structured follow-up; honest surgeons will decline patients for whom the risks outweigh the benefit.

What is the difference between Smart Lens and LASIK?

LASIK reshapes the cornea to correct focus and is best for younger patients with healthy natural lenses, whereas Smart Lens replaces the lens itself. Smart Lens corrects presbyopia and high prescriptions that LASIK cannot fully address, and it removes the future risk of cataracts — which is why it is usually recommended from around age 45.

Will I still need glasses after Smart Lens?

Most patients greatly reduce or eliminate their need for glasses, and with trifocal or EDOF lenses many read, drive and use screens without them. Some still prefer glasses for very small print or long night drives, and no surgeon can promise total spectacle freedom in every situation.

Is Smart Lens permanent?

Yes. The artificial lens does not age, cloud or wear out, so the correction is permanent and you can never develop a cataract in the treated eye. If a residual prescription remains it can be fine-tuned, and a separate clouding of the lens capsule that can occur later is treated in minutes with a YAG laser.

Does the surgery hurt?

No. The procedure is done under anaesthetic eye drops, often with light sedation, so you feel pressure or movement rather than pain. Mild grittiness, watering and light sensitivity for a day or two afterwards are normal.

How long do I need to stay in Turkey?

Plan for about 5-7 days. This allows your pre-operative assessment, treatment of each eye on separate days and at least one post-operative check before you fly home, with remaining follow-up coordinated remotely.

When can I fly and go back to work?

Flying is usually fine a day or two after your final check, and many people return to desk-based work within a few days, avoiding dusty or strenuous environments for a week or two. Your surgeon gives you personalised timings based on your healing.

Am I too short-sighted or long-sighted for Smart Lens?

Smart Lens can correct very high prescriptions that are beyond the safe range of laser eye surgery, which is one of its main advantages. The only way to be sure is a full assessment of your cornea, retina and lens — and for very short-sighted eyes the surgeon will also weigh the slightly higher retinal-detachment risk.
