Overview

An echocardiogram ("echo") is a painless ultrasound scan of the heart that lets a cardiologist see your heart muscle, valves, chamber sizes and pumping strength in real time. In Turkey it is widely available at accredited cardiology centres, usually as a same-day outpatient test that is read and reported by a cardiologist, often bundled with a consultation. An echocardiogram uses high-frequency sound waves (ultrasound) to create moving images of your heart. Because it uses sound rather than X-rays, there is no radiation involved, which makes it safe to repeat as often as needed and suitable for almost everyone, including pregnant patients. It is one of the most important tests in cardiology for diagnosing and monitoring heart conditions.

The scan shows the size and thickness of the heart's four chambers, how well the muscle squeezes (the ejection fraction), whether the four valves open and close properly, the pressure inside the heart and lungs, the lining around the heart (pericardium), and any structural or congenital abnormalities. Doppler ultrasound is added to measure the speed and direction of blood flow through the valves.

How it’s done

For a standard transthoracic echocardiogram (TTE) - the most common type - you lie on your side on a couch, a small amount of gel is applied to your chest, and the sonographer or cardiologist moves a hand-held probe (transducer) across the chest and below the breastbone. Small stickers (ECG leads) record your heartbeat at the same time. The test is completely painless and usually takes 20 to 45 minutes. You can eat, drink and take your medicines as normal beforehand.

Other forms are used when more detail is needed. A transoesophageal echocardiogram (TOE) passes a thin probe down the gullet after throat numbing and light sedation, giving very clear pictures of the valves and the back of the heart. A stress echocardiogram images the heart before and after exercise on a treadmill or bike, or after a medicine (dobutamine) that makes the heart work harder, to look for reduced blood supply. A contrast or "bubble" study injects agitated saline or a contrast agent into a vein to outline the chambers or detect a hole between them.

Is it right for you?

An echocardiogram is a diagnostic test rather than an elective procedure, so it is ordered when there is a reason to look at the heart - symptoms such as breathlessness, chest pain, palpitations, dizziness or fainting; a heart murmur; known or suspected valve disease, heart failure, cardiomyopathy or congenital heart disease; before certain operations; or to monitor the heart during chemotherapy. It is safe for almost everyone, including children and pregnant women, because there is no radiation. The main limitations are technical: image quality with a standard TTE can be reduced in people with a large body habitus, significant lung disease or chest deformity, in which case a transoesophageal echo or another imaging test may be advised. A TOE is generally avoided in people with certain swallowing or oesophageal problems, and a stress echo is not suitable for those who cannot exercise or have an unstable cardiac condition.

Recovery & aftercare

A standard transthoracic echocardiogram needs no recovery at all - you wipe off the gel, get dressed and resume all normal activities, including driving, immediately. There are no restrictions on eating, working or exercise.

A transoesophageal echo (TOE) involves light sedation and throat numbing, so you fast beforehand, rest until the sedation wears off, and may have a mildly sore throat for a day; you should not drive or travel alone for the rest of that day and need someone with you. After a stress echo you simply cool down and recover from the exertion before going home, usually within an hour.

Results to expect

Because the cardiologist watches the heart moving in real time, key information is often available straight away, with a full written report - including ejection fraction, valve gradients and chamber measurements - typically issued the same day or shortly after. The echo can confirm or rule out conditions such as heart failure, valve disease or a cardiomyopathy, quantify how severe they are, and provide a baseline to compare against future scans. It does not, on its own, diagnose every heart problem - blocked coronary arteries, for example, may need additional tests such as a stress echo, CT or angiography - so results are always interpreted alongside your symptoms and other findings.

Safety & honest risks

A standard transthoracic echocardiogram is one of the safest tests in medicine, with essentially no risk - ultrasound has no known harmful effects and there is no radiation. The more specialised forms carry small, well-understood risks that good centres manage carefully. A transoesophageal echo can cause a temporary sore throat and, very rarely, injury to the gullet, bleeding or a reaction to the sedation; it is performed by experienced operators with monitoring and emergency equipment to hand. A stress echo can occasionally provoke an irregular heartbeat, chest pain or, very rarely, a more serious cardiac event, which is why it is supervised by a cardiologist with resuscitation facilities available. Contrast agents are very well tolerated but can rarely cause an allergic reaction.
